ELECTROENCEPHALOGRAPHY (EEG)

 

EEG is an examination of spontaneous brain activity. Being one index of cerebral function, this test is relevant not only in neurology but also in other specialties. EEG can be done to diagnose various neurological disorders and also to evaluate the response of treatment.  This test may be helpful in prognostication of various disorders.

 

EEG may be a useful diagnostic indicator in relation (but not limited) to the following diseases:

  • Seizure disorders (Location of the focus, type of seizure disorder)

  • Evaluation of transient spells (Transitory Ischemic Attack TIA, Residual Ischemic Neurological Deficit RIND, Syncope)

  • Intracranial disease process (encephalitis , tumor, abscess, Cerebrovascular disorders )

  • Diffuse disturbances of cerebral function (metabolic & Endocrine diseases, and degenerative process, eg. Creutzfeldt - Jakob disease)

  • Coma (status epilepticus, drug overdose, hepatic coma)

  • Non-epileptic paroxysmal disorders

  • Psychosomatic diseases

 

 

ELECTROMYOGRAPHY (EMG) &

NERVE CONDUCTION STUDIES (NCV)

 

EMG /NCV studies are done to detect and evaluate damage the peripheral nervous system, which includes all the nerves that lead away from the brain and spinal cord and the smaller nerves that branch out from those nerves. Nerve conduction studies are often used to help diagnose nerve disorders, such as carpal tunnel syndrome or Guillain-Barre syndrome. It can also be done to identify the location of abnormal sensations, such as numbness, tingling, or pain.

NCV/EMG is helpful in diagnosis and evaluation of symptoms like:

  • Radiculopathy  / Nerve root Compression

  • Neck Pain/ Back pain

  • Carpal Tunnel Syndrome

  • Wrist pain

  • Tarsal tunnel syndrome

  • Foot pain

  • Mononeuritis multiplex

  • Burning  & Tingling  sensation

  •  Peripheral neuropathy

  • Numbness

  • Wrist Drop, Foot drop

  • Weakness

  • Neuralgia/Neuritis/ Radiculitis

  • Pain in limb

  • Brachial  Plexopathy

  • Shoulder  pain

  • Lumbosacral Plexopathy

  • Thigh pain

  • Muopathy

  • Muscle weakness

  • Myalgia/Myositis

  • Muscle pain

  • Fasciculations

  • Thinning of limbs

  • Lumbar IVD w/o Myelopathy

  • Spinal stenosis

  • Thoracic Outlet Syndrome

  • Cervicobrachial Syndrome

 

 

Contraindications for NCS

  • Patients with pacemaker, Swan-Ganz catheter

  • Wounds or infection at site to be tested

  • AV fistula at site to be tested

 

Contraindications for Needle EMG

  • Ongoing anticoagulant therapy (e.g. warfarin / coumadin, heparin, etc.)

  • Active bleeding and bleeding disorders (e.g. thrombocytopenia, hemophilia, etc.)

  • Wounds or infection at site(s) to be tested

  • Patients who are susceptible to developing infections (e.g. valvular heart disease, prosthetic valves, endocarditis)

  • Patients with pacemaker, Swan-Ganz catheter

 

EVOKED POTENTIAL STUDIES

 

Visual Evoked Potentials (VEP)

 

The VEP tests the function of the visual pathway from the retina to the occipital cortex. It measures the conduction of the visual pathways from the optic nerve, optic chiasm, and optic radiations to the occipital cortex.

Some of the symptoms for which VEP is done may include:

  • Loss of vision (this can be painful or non-painful);

  • Double vision

  • Blurred vision

  • Flashing light

  • Alterations in color vision

 

Brainstem Auditory Evoked Potential (BAEP/BAER)

 

Brainstem auditory evoked response (BAER) or brainstem auditory evoked potential (BAEP) is a hearing test that detects electrical activity in the cochlea and auditory pathways in the brain. BAEPs are useful in estimating or aiding in the assessment of hearing loss. In children, especially those younger than 2 years, the BAEP can be used to screen those who might benefit from auditory amplification in order to achieve more normal speech and language development.

 

Some of the symptoms and disorder for which BAER may be helpful: 

  • Hearing loss

  • Classification of type of deafness (conductive or sensorineural)

  • Unilateral tinnitus

  • Unilaterally or bilaterally poor word recognition scores as compared with degree of sensorineural hearing loss

  • Perceived distortion of sounds when peripheral hearing is essentially normal

  • Retrocochlear pathology such as an acoustic neuroma or vestibular schwannoma (Brain Tumor)

 

 

REPETITIVE NERVE STIMULATION TEST (RNST)

 

Repetitive nerve stimulation (RNS) is important confirmatory tests for the diagnosis of disorders of the nicotinic neuromuscular junction, particularly for myasthenia gravis, Lambert-Eaton myasthenic syndrome, and botulism.

 

RNS (Rapid Nerve Stimulation) Test is done for evaluation of symptoms like:

  • Weakness & Fatigability

  • Ptosis (Drooping of eyelids)

  • Diplopia (Double vision)

  • Dysphagia (Difficulty in swallowing)

  • Dysarthria (Slurred speech)
